How to Arrange Transportation After a Hospital Discharge in New Hampshire

Getting a patient home safely after discharge requires the right vehicle and advance planning. A complete guide for NH families and care coordinators.

One of the most stressful moments for families is the call from the hospital: your loved one is being discharged. The medical team has done their work. Now you need to get the patient home, or to a rehabilitation facility, safely and on time. For many patients, that transition is far more complex than simply calling a family member or hailing a rideshare.

This guide walks you through everything you need to know about arranging transportation after a hospital discharge in New Hampshire, including what type of transport is right for the patient, how to coordinate with hospital staff, and how to book a qualified NEMT provider.

Why Hospital Discharge Transportation Is Different

Discharge transportation is not the same as arranging a ride to an outpatient appointment. Patients leaving a hospital may be:

  • Unable to sit upright due to surgery or a recent procedure
  • On oxygen or requiring monitoring during transport
  • Using a wheelchair or walker for the first time after an injury

Wheelchair Transport

For patients being discharged in a wheelchair, whether temporarily after surgery or on a permanent basis. The vehicle must have a hydraulic lift or a ramp and four-point tie-down restraints. The patient stays in their wheelchair for the entire trip. Do not book a standard vehicle if the patient will arrive at discharge in a wheelchair.

Stretcher Transport

For patients who cannot sit upright. This includes patients recovering from abdominal surgery, severe back injuries, certain cardiac procedures, or any condition where maintaining a reclined position is medically required. Stretcher transport is the appropriate alternative to an ambulance when the patient is stable but cannot sit up.

Stairchair Transport

For patients going home to a residence without elevator access and who cannot safely navigate stairs independently. A motorized stairchair device allows trained staff to move the patient up or down stairs without risk of falls or injury.

Step 3: Coordinate with Hospital Staff

The hospital discharge coordinator or case manager is your primary contact for logistics. They can provide:

  • The exact discharge date and approximate time window
  • Medical clearance documentation the transport provider may need
  • Information about any equipment the patient will be taking home
  • Contact information for the receiving facility if this is a facility-to-facility transfer

Step 4: Book the NEMT Provider in Advance

Do not wait until the morning of discharge. Hospital discharge times can shift by hours, and last-minute NEMT bookings carry more risk of delay. Aim to book at least 24 to 48 hours before the anticipated discharge time.

When booking, provide:

  • Patient name and date of birth
  • Pickup location: hospital name, ward, and room number if known
  • Destination address, home or receiving facility
  • Anticipated discharge date and time window
  • Type of transport needed: ambulatory, wheelchair, stretcher, or stairchair
  • Any special requirements such as oxygen on board or companion seating

Step 5: Prepare the Destination

Before the patient arrives home, make sure the space is ready:

  • Clear pathways from the door to the bedroom and bathroom
  • Arrange any rented medical equipment, such as a hospital bed or shower chair, to be in place before arrival
  • If the patient uses a wheelchair, confirm the entry to the home is accessible or that stairchair service has been arranged

What to Do If Discharge Happens Faster Than Expected
